How to read your B12 blood test, and where 500 mcg fits

Start With Your Lab Report

If you have had a vitamin B12 test, the result is usually called serum vitamin B12 and is reported in pg/mL (picograms per millilitre) or pmol/L. To convert, multiply pg/mL by 0.74 to get pmol/L.

Commonly used reference ranges (they differ between labs, so always check the range printed on your own report):

Serum B12 (pg/mL)General interpretation
Below about 200Commonly considered low
About 200 to 300Borderline; a doctor may order further tests
Above about 300Commonly considered adequate

Why "borderline" needs more tests: serum B12 does not always reflect how well your cells are using B12. Doctors may also check methylmalonic acid (MMA) or homocysteine, which rise when B12 is not sufficient. Your doctor will tell you which tests make sense for you.

Important: these ranges are a guide for understanding your report. They are not a diagnosis. A low result should be discussed with a doctor, since the cause matters as much as the number.

What B12 Does

Vitamin B12 contributes to:

  • Normal energy-yielding metabolism
  • Normal functioning of the nervous system
  • Normal red blood cell formation
  • Normal psychological function
  • Normal homocysteine metabolism
  • The reduction of tiredness and fatigue
  • The normal function of the immune system

Why Some People Have Low B12

  • Little animal food. B12 comes almost only from meat, fish, eggs and dairy, so vegetarians and people who eat these foods rarely are at higher risk.
  • Age. Stomach acid declines with age, which reduces B12 absorption from food.
  • Medicines. Long-term metformin and acid-reducing medicines can lower B12 levels.
  • Digestive conditions or stomach surgery.

The B12 Strength Ladder

StrengthPosition
Around 2.4 mcgDaily requirement for adults
100 to 250 mcgLow-dose supplement, common in multivitamins
500 mcgEveryday supplement strength
1,000 mcgCommon higher-strength supplement
2,000 to 5,000 mcgHigh strength

Why supplements use hundreds of micrograms when you need only 2.4: at larger doses, only a small share of B12 (roughly 1 percent) is absorbed by simple diffusion, without needing the stomach protein intrinsic factor. Supplement strengths are designed around that small share, and what is not absorbed is passed out of the body. No upper limit has been set for B12.

Ingredient Breakdown

NutrientAmount per servingForm
Vitamin B12500 mcg[cyanocobalamin / methylcobalamin, confirm on label]
Other ingredients[copy from label]

Format: [tablet / capsule / chewable / lozenge, confirm]
Serving size: [as per label]

About the forms: cyanocobalamin is a stable synthetic form that the body converts, and it is the most studied form. Methylcobalamin is an active coenzyme form. Both can raise B12 levels in people who are low, and research comparing them is limited, so we do not claim that one is always better. Use the form your label states.

How to Take It

  1. Take [serving as directed on the label].
  2. Many people take it in the morning or with lunch, because some find B vitamins slightly energising.
  3. Take it consistently. B12 supports levels over weeks.
  4. Retest as your doctor advises, often after a few months, to see how your level is changing.

Safety Notes

  • B12 is generally well tolerated. Rarely, people notice mild stomach upset, headache or skin reactions.
  • If you have a confirmed deficiency, do not rely only on over-the-counter supplements. Your doctor may recommend a specific dose or injections.
  • Tell your doctor about all your medicines and supplements.
  • Ask a doctor before use if you are pregnant or breastfeeding or have a health condition.
  • Keep out of reach of children.

Yes. You can order it from our store with delivery to cities across Pakistan.

Commonly, above about 300 pg/mL is considered adequate, but ranges differ between labs. Your doctor will interpret your result.

It means your doctor may order further tests, such as MMA or homocysteine, to see whether your body is short of B12.

At larger doses, only a small percentage of B12 is absorbed. Supplements use larger amounts so that a useful quantity gets through.

Vegetarians, older adults, people taking metformin or acid-reducing medicines, and people with certain digestive conditions.

No. Plant foods do not reliably provide B12.

Morning or lunchtime works for most people, with or without food.

No. It supports normal energy metabolism, but it is not a stimulant.

Many people do. Ask your doctor, since metformin can lower B12 over time.

Yes. Testing shows your level and helps you and your doctor decide what to do.
